Health and wellness assessments are performed by dedicated students in Walsh University’s Doctorate of Physical Therapy and Master of Occupational Therapy programs. All check-ups are supervised by licensed physical therapists, occupational therapists and nurses. Please print and complete the Clinic Appointment Intake form (PDF) prior to your appointment.

The Annual Check-Ups Include:

  • Free health and wellness screening through movement, strength and balance assessments
  • Referral to community resources or additional healthcare providers based on individual needs
  • Previous attendees: compare this year’s health, wellness, and assessment scores to data from previous years and receive individualized education from nursing, physical therapy and occupational therapy students
